Disease association and antibody prevalence
- 40% of patients with selective IgA deficiency
- 10 to 25% of patients with Common Variable Immunodeficiency (CVID)
Selective IgA deficiency is most common in celiac disease, while CVID is the most common symptomatic primary immunodeficiency (prevalence ~ 0.004%) characterized by low serum levels of IgG, IgA and variably IgM.
Prevent anaphylactoid reactions
Immunoglobulin G (IgG) replacement therapy represents an effective prophylaxis for infections in CVID patients. However, in any given IgG product, traces of IgA (0.05 to 6.0 mg/dl depending on the product) can be found. Therefore, patients with anti-IgA antibodies are at risk to develop an anaphylactoid reaction to these IgG formulations, especially upon intravenous andministration. The frequency of transufsion reactions is approximately 1 in 30,000 in the general population, while it is as high as up to 1 in 30 in IgA deficiency.
When is the measurement recommended?
- Mandatory:
Patients who have IgA deficiency and are destined for blood transfusion or IgG replacement therapy.
- Recommended:
Celiac disease patients who are IgA deficient in order to assess a future risk for blood transfusions.
- Useful:
Any IgA deficient patient in order to assess a future risk for blood transfusions.
